    Fix TX-slot termination self-deadlock

    Defer fatal errors discovered while draining or gathering channel TX
    slots until after channel_mtx is released. This prevents session_set_terminate() from reacquiring the same non-recursive mutex
    while it wakes connection-layer waiters.

    Add failure-injection coverage for both per-packet and gathered send
    paths, with CTest timeouts to turn regressions into bounded failures.
    Record the affected Coverity CIDs as fixed pending rescan.
